Evans School degree project forms are available in PDF format and require Adobe Acrobat Reader to view.
- Degree Project Proposal Form (34 KB PDF): Must be submitted to the Student Services office before getting a registration code
- Degree Project Sample Title Page (5 KB PDF): Sample format for the title page of your degree project
- Degree Project Summary (12 KB PDF): Must be submitted with your degree project
